If you are asked to be a sponsor (godparent) for the Sacrament of Baptism or a sponsor for the Sacrament of Confirmation, most parishes require that the proposed sponsor obtain a Sponsor certificate (also called a Letter of Eligibility) from the parish that they currently belong to and attend Mass on a regular basis.

Every parish has a form which may be completed by its parishioners, if they meet the requirements to be a sponsor.  Some parishes (in which the sacrament is to be administered) have their own form, which they require to be signed by a priest of the parish of the proposed sponsor.
